APHIS / CDC Form 2 - Request to Transfer Select Agents and Toxins
Description
A registered entity is required by regulation (7 CFR 331.16, 9 CFR 121.16, and 42 CFR 73.16) to file this form with either APHIS or CDC and obtain approval prior to transfer of a select agent or toxin. The purpose of this form is to provide a method for the documentation of the transfer of a select agent or toxin. The form must be completed for each transfer of select agents.
